This form should be completed within 30 Days for expenses incurred by an Oaks of Edgewood HOA member/volunteer assisting with HOA activities and/or business.  

Please submit this form and attach receipts for any expenditures that require reimbursement from the HOA.  Each person who will be receiving a check must submit a copy of this form.
